Guest Speaker Gift Card Requests Process

  1. Faculty member fills out the following Class Guest Speaker Request Form and sends to Associate Dean for approval (this is a fillable PDF form, please download a copy).
  2. Once approved, the AD forwards the form to the Operations Manager, Finance & Resources (Affaf) for disbursement.
  3. Faculty members are required to confirm guest speaker attendance prior to issuance of gift card.
  4. Operations Manager, F & R e-mails a gift card directly to the guest speaker with a copy to the requesting faculty member (please allow up to 3-4 business days for processing gift requests).

Class Guest Speaker Request Form

  • Please submit the form at least 1 week prior to the scheduled class/event.
  • Please submit 1 form per class.
  • If you require this form in an alternative format, please contact affaf.zahid@humber.ca
  • There is an option to gift complimentary thank you cards to in-person guest speakers. For complimentary thank you card requests, faculty members will be notified via e-mail when the card is ready for pick-up from Front Office (C205).
