Please submit the form at least 1 week prior to the scheduled class/event.
Please submit 1 form per class.

Once approved, the AD forwards the form to the Operations Manager, Finance & Resources (Affaf) for disbursement.
Faculty members are required to confirm guest speaker attendance prior to issuance of gift card.
Operations Manager, F & R e-mails a gift card directly to the guest speaker with a copy to the requesting faculty member (please allow up to 3-4 business days for processing gift requests).
There is an option to gift complimentary swag bags to in-person guest speakers. For complimentary swag bag requests, faculty members will be notified via e-mail when the gift is ready for pick-up from Front Office (C205).
